Hello, my name is Tara Bukowsky RN, BSN, CCM.

I established Clear Care Transitions, LLC to serve as your personal compass and be a strong advocate through the chaotic maze of healthcare decisions and transitions.

Service Agreement

The following outlines the terms and scope of services provided by Clear Care Transitions, LLC and is provided here for your review.  We are happy to answer any questions you may have by calling 479-250-6636.

 

This agreement is designed to ensure clarity, transparency, and a shared understanding Consultant's role and services.

Please review this information carefully prior to engaging services.

Should you desire to initiate services, an electronic copy of this agreement will be sent to you via our secure platform for signature, a secure payment link and a copy our HIPPA Release of Information for medical records. 

Healthcare Transition Consultant Service Agreement

This Agreement is entered into between: Clear Care Transitions, LLC. Consultant: Tara Bukowsky RN, BSN, CCM – Consultant

Client – Whether one or more:

 

1) Scope of Services

Consultant provides time-limited, consultative services to assist in understanding healthcare situations and care transitions.

Services may include the provision of information, resources, education, and general guidance.

Services may include:

• Review of medical situations and care plans

(Including but not limited to clarification of changes in condition, care needs, or level of care, and what those changes may mean for next steps)

• Explanation of healthcare options

(Including but not limited to home, rehabilitation, skilled nursing, or long-term care)

• Discharge plan interpretation

(Including but not limited to explanation of discharge recommendations and what they may mean in practical terms)

• Identification of potential concerns or gaps in care

(Including but not limited to highlighting areas that may impact care or safety)

• General guidance to support informed decision-making

(Including but not limited to providing objective input to help support clear and informed decisions)

 

Consultant provides recommendations for consideration only.

Final decisions remain he sole responsibility of Client, family members, and/or designated caregivers.

 

At no time will Consultant provide medical advice or diagnoses.

 

2. Services Not Provided

 

Consultant does NOT and will NOT provide:

• Direct patient care or clinical treatment

• Emergency or urgent medical services

• Ongoing case management or care coordination

• Legal, financial, or medical advice

• Decision-making on behalf of the Client

• Arrangement or scheduling of care services

 

Consultant’s role is strictly consultative.

Any actions taken based on information or guidance provided are the responsibility of Client and / or their designated representatives.

3. Initial Pre-paid Consulting Retainer:

Services are provided on a prepaid basis.

 

An initial prepaid retainer of $240.00 is required before services begin.

This retainer is applied toward all professional services provided, including the healthcare transition consultation, review of available information, research, education, guidance, documentation, and follow-up communication related to the agreed scope of services.

 

Professional services are billed at $120.00 per hour, with a one-hour minimum for the initial consultation.

Thereafter, time is billed in 15-minute increments and deducted from the remaining prepaid balance.

When the prepaid balance has been exhausted, additional prepaid funds will be requested before services continue.

Services will begin only after payment is received and will continue only while a prepaid balance remains available.

Services will continue only after subsequent payment is received.

All fees are non-refundable once services begin, except in cases where prepaid services have not yet been rendered.

Any unused portion of prepaid fees may be refunded at Consultant’s discretion.                                                                        

 

4. Communication and Availability

 

• Services are provided via phone or video consultation unless otherwise agreed

• Communication is limited to scheduled consultations and reasonable follow-up as determined by Consultant

• Consultant is not available for continuous or on-demand support

Clients may expect:

• A structured consultation

• Clear explanation of options

• Follow-up communication as appropriate to the agreed scope of services

 

5. Term and Cancellation

 

• This Agreement begins on the signed Service Start Date

• Either party may terminate this Agreement at any time without cause

If terminated:

• Client is responsible for payment of all services rendered up to the termination date

• Any unused prepaid balance may be refunded as outlined above

 

6. Limitation of Role and Liability

 

Consultant acts solely in a consultative and advisory capacity, providing information and guidance for educational purposes only.

While every effort is made to provide accurate and relevant information, no guarantees are made regarding outcomes.

Healthcare situations are dynamic, and services are based on information available at the time of consultation, which may change as circumstances and resources evolve.

Client acknowledges that all decisions and actions remain their sole responsibility.

Consultant is not responsible for the actions or omissions of healthcare providers, other third parties, or for decisions made by Client and/or their caregivers.

Implementation of any recommendations or guidance is the responsibility of Client and/or their designated representatives.

 

7. Choice of Venue

Any litigation arising from this Agreement shall be heard in the Circuit Court of Benton County, Arkansas.

Arkansas law governs this Agreement.

 

8. Entire Agreement

This Agreement constitutes the entire agreement between the parties hereto with respect to the subject matter contained in this Agreement and supersedes all prior agreements, understandings and negotiations between the parties.

 

9. Singular and Plural

Words used herein regardless of the number and gender specifically used, shall be deemed and construed to include any other number, singular or plural, and any other gender, masculine, feminine or neuter, as the context requires.

 

10. Headings

Headings are for ease of reading only and shall not be used to interpret this agreement.
